For appointments*:
Please email or fax your "New Patient Form", insurance card, recent labs and results, and Photo ID prior to your appointment time, or bring them to your appointment. We do not offer urgent or emergency care.
Diagnostic tests and labs are through Quest labs and local hospitals and medical centers; heart monitors are applied in our office and results read by our PA and one of our in house cardiologists.

We work to help people get off of ADHD meds, benzodiazapines and opiates, via safe medically guided deprescribing. We do not start people on theses medicines and do not perform disability or medical leave assessments.
